Hospitals with over 200 beds 
Universities

Private Industry with over 500 employees 

Institutional Membership Application

Contributions are tax deductible.

Gold Level

$500 

Hospitals with over 100-199 beds 
Community Colleges 
Surgery Centers 
Private Industry with between 300 and 499 employees 

Institutional Membership Application

Contributions are tax deductible.

Silver Level

$250 

Adult Education 
Government Agencies 
Hospitals with 99 or fewer beds 
Large Retirement Communities 
Private Industry with between 100 and 299 employees
