The San Joaquin Valley Health Consortium has two different membership application types. These applications are provided in PDF format. This format will allow you to download the membership application to your computer. To do so you must have the Adobe Acrobat Reader installed on your computer. If you do not have this program you can download it free of charge from Adobe.

The San Joaquin Valley Health Consortium was founded in 1972. Our mission throughout the San Joaquin Valley is to improve healthcare. This is done by taking a proactive role to identify needs, acquire resources, coordinate the use of resources, disseminate information, promote health education and respond to health opportunities.
